The plan for Simon and The Saviors to infect a few members of The Hilltop by hitting them with weapons covered in walker guts got adjusted when Negan disappeared and killing became a higher priority.
Survivors at The Hilltop were ready for the assault. Despite them taking some casualties and being wounded by some of the weapons, they were able to mount a good defense against Simon’s group, which included Derek.
Derek was successful in finding Father Gabriel and Dr. Carson during a previous mission, but this altercation wasn’t as fortuitous for him, as he followed the orders of Simon with other members of The Saviors to attack.
Carol spotted him attacking Tobin inside the fence of The Hilltop and took the shot. She was able to kill Derek, but not before he cut Tobin with the infected weapon.
